BEP/HDP Client Consent
Form
To print
the necessary Client Consent Form, we recommend you SAVE the required
file in your hard drive or RUN the program from the website. Each
time you print this form, a unique barcode will be generated for each copy.
This barcoded form will be used to scan your data into our system.
SAVE:
After you have clicked "Save", the file will be copied in your hard
drive, so remember where you have saved it. Go to Windows Explorer,
then go to the folder where you have saved the file and double click
on the file called BIOForms.exe.
From here you can access the Client Consent Form by viewing or printing it.
To print more forms for later use, you do not have to save the file again.
All you have to do is reopen the BIOForms.exe from you hard drive.
RUN:
After you have clicked "Run", you may get a window security pop-up, click
RUN again. From here you can access the Client Consent Form by viewing
or printing it.
